Treatment session notes a chiropractor provides should record the following information:
1) The patients name and DOB.
2) A record of the patients subjective complaint.
3) The Chiropractors objective findings, including screening tests ruling out more serious problems.
4) Tests performed specific to the patients disorder. Also any flags that maybe affected by treatment.
5) The chiropractors assessment of the patients condition.
6) A working diagnosis of the problem.
7) The types of therapy provided and the areas of the body the treatment was administered to.
8) The time interval to the next appointment.
9) The advice given regarding rest, exercise etc.
10) A signature from the chiropractor together with the date of the treatment session
As you will be aware note taking in many areas of the medical profession is often below par, consequently healthcare professionals are often reprimanded by their managers/governing Associations for inadequate note taking, case history taking tends to be very good (the Case history and examinations carried out by Chiropractors are of a very high standard) but health practitioners everyday note taking of soap notes (ie. treatment sessions) is sometimes substandard in both content and very often the notes are only legible to the practitioner who wrote them.
As everybody treats differently it is almost impossible to provide a set of standardized SOAP notes suitable for everybody.
